Care Management

The Yukon-Kuskokwim Health Corporation provides patients with a Case Manager. Case Managers work with physicians and other providers to help people make decisions about health care. They may assist with Inpatient and Outpatient services, referrals to and work with Social Services to arrange for continued care after discharge. Case Managers may also assist the Utilization Review Coordinator with inquires to and from insurance and other third party payers to give them information needed to provide coverage for the patient. They may also assist patients without coverage to seek funds to assist with payment for their illness or injury or transportation.

Case Managers are available for Kusko Clinic, Yukon Clinic, Obstetrical Services, Specialty Clinic, Oncology, Inpatient, OR and Women’s Health.

Laboratory Services

The Laboratory Department provides acute and out-patient testing services and reports to all clinicians and departments associated with the Yukon-Kuskokwim Health Corporation for patients ranging in age from newborn to geriatric. Testing services and their associated reports are also provided upon written request for those non-YKHC physicians within the State of Alaska with non-beneficiary clients on the Delta. The Laboratory department interfaces with other accredited health care organizations for those tests not analyzed in-house.

Laboratory services consist of hematology and coagulation; serology; microbiology; DNA probes; chemistry, special chemistry and blood gases; urinalysis, screening for drugs of abuse; blood bank; venipuncture, clerical, sendouts; support of the Sub-Regional Clinic and support of Point of Care Testing within the hospital and CHA/Ps. The CHA/Ps draw lab specimens and forward them for processing at the lab at YKDRH. The Laboratory is open 24 hours a day, 7 days a week.

Radiology/ Diagnostic Imaging

Upon receipt of a study requested by an authorized provider, the Diagnostic Imaging Department provides service to any registered patient of YKHC. We do general x-ray exams, computed tomography, contrast studies, mammography and sonograms. Services are provided to all age groups and they are predominantly outpatients.

The department is open from 8:00am to 7:00pm to support Outpatient Clinics and staffed 24/7 with a Technologist able to perform general and computed tomography exams, to support the Emergency Department.

The department is staffed by AART licensed technologists approximately 24 hours a day, 7 days per week (except in extenuating circumstances, when an on-call technologist is designated) for general radiology and computed tomography. An RDMS licensed sonographer performs sonograms 9 hours a day, Monday through Friday. Certified Mammographers perform mammograms by appointment and walk-in Monday through Friday. Certified Mammographers perform mammograms by appointment and walk-in Monday through Friday. ACR board-certified Radiologists provide interpretation of all studies.

Pharmacy

Outpatient Services

Medical history review, interviewing patients for medication history, filling of prescriptions, patient counseling/education including food/nutrient interactions, cytotoxic IV preparation, distribution of medications to village and subregional clinics, drug information.

Inpatient Services

Preparation of sterile products (IV admixtures, piggybacks, syringes), medication histories, monitoring of drug therapy, drug information, discharge counseling/education on medications including food/nutrient interactions.

Hospital Community Relations

Some of the major duties of this department include providing interpreter services for our Yup’ik speaking patients and providers, and assisting handicapped and Elders throughout the hospital. We also serve as the initial coordinating link and contact person between patients and other areas of the hospital by acting as greeters, providing information to patients and providers on contract health issues and assisting patients with filling out Patient Comment forms, as well as locating wheelchairs and security when necessary for patients.

The Hospital Community Relations department operates 11 hours a day, Monday through Friday, 8:00 AM to 7:00 PM. After hours, weekends, and holidays, personnel are on call. /Translation

Dental

The Dental Department provides a broad range of dental treatment services in Bethel including emergency services, preventive, and basic restorative services. In Bethel we are able to provide root canal treatment and selected cases. The Bethel clinic also provides crown, bridge, and denture services. As in any dental practice some complicated cases have to be referred to specialists. We now have 8 full time Dental Health Aide Therapists (DHATs) so all of the sub regional clinics have a full time DHAT who offers emergency, preventive, and basic restorative services. Our dentist and DHATs make periodic visits to all the 48 villages in the YK Delta.

23 Services in Bethel are provided in the Community Health Services Building (CHSB) dental clinic and in the YKDRH operating room. Community prevention projects are an important aspect of the program. The hours of operation are Monday – Friday 8:00am – 12:00pm, 1pm – 5:00pm. Patients range in age from infants to elderly.

Walk-ins or emergency patients are assessed for their presenting chief complaint. Appointment patients are given an oral examination and x-rays. Based on these assessments, a treatment plan is developed for the patient.

The Dental Department is staffed with 13 full time dentists, 8 Dental Health Aide Therapists (Subregional Clinics based), 2 dental hygienists; 1 Primary Dental Health Aide coordinator, 8 Primary Dental Health Aides, 19 dental assistants; and 5 administration staff. A dentist is assigned to be "on call" for after hours emergencies (via the hospital ER).

Optometry Department

The Optometry Department provides routine eye exams; treatment and management of ocular diseases, i.e. glaucoma, diabetes, and red eyes; diagnosis of ocular abnormalities; follow-up care for surgeries; follow-up care for glaucoma/diabetes, i.e., fundus photo/visual field; scheduling, glass repair, and ordering for patients; provide specialty clinic four times per year; 30 village trips per year to provide eye care; educational needs related to eye care. Patients range in age from pediatric to geriatric.

Hours of operation are Monday through Friday, from 8:00 AM to 5:00 PM. Patients are scheduled between 8:00 AM to 5:00 PM.

Behavioral Health

The Mental Health and Substance Abuse and Developmental Disabilities service delivery system at YKHC is comprised of various service components. The components are Crisis Services, Outpatient and Village Services, Residential Services, Developmental Disabilities, and Support Services.

Crisis Services

Crisis Services will respond to urgent behavioral health services needs that occur in the YK regional and in the YKHC hospital. Urgent behavioral health services needs include responding to Title 47 requests for mental health status evaluations to determine what level of care a patient will need to alleviate an urgent behavioral health crisis. Urgent care walk-in times and primary care consultation services are also provided.

Emergency Services

Conduct behavioral health evaluations of individuals who present as being in crisis and to take appropriate steps to initiate the provision of services depending on the outcome of the evaluation. Will assist monitoring of patients, in the hospital northwing, who are involuntarily held for Title 47-related observation and evaluation for further treatment or referral. Village Services directly administers the Emergency Services component. Emergency On-Call Responders-Emergency Services Clinicians and Complex Care Managers are available twenty-four hours per day to respond to behavioral health crisis for all individuals in the Yukon-Kuskokwim service area. The responders provide immediate mental status examinations and work with the primary care providers in the village or hospital to plan care for management of the crisis and follow-up of the individual. On-Call Attendants are available to stay with an individual admitted to the hospital for evaluation when ordered by an admitting physician to provide safety and security for a client. During the evaluation period comprehensive mental health and substance abuse assessments are completed by the Emergency Services Clinician to assess and manage the client within the behavioral health system of care. The Emergency On-Call Response Team is available twenty-four hours per day at 543-6100 or 1-800-478-2642

Crisis Incident Stress Debriefing

Coordinate Critical Incident Stress Debriefings (CISD) response teams with other providers throughout the YK –Delta and debrief with trauma first responders. Develops Crisis intervention teams with community partners throughout the YK-Delta. Coordinates the training and certification of the crisis team members. Coordinates all logistics of crisis village-based responses by contacting and deploying team members and ensuring adequate coverage and follow-up.

Prevention Services

Prevention services, established in January 2012, has established traditional healing and elder consultation as the forums with which to implement community and family support systems in the region’s villages.

Family Spirit Program

The Family Spirit Program grew out of a collaborative effort of the communities of the Yukon-Kuskokwim region, the Department of Health and Social Services, Division of Alcohol and Drug Abuse, Office of Children Services, the YKHC, Association of Village Council Presidents (AVCP), Tundra Women’s Center (TWC), Orutsararmuit Native Council (ONC) and other community providers in the Yukon-Kuskokwim Delta area. The primary purpose of the project is to implement a community development model that strengthens families, using collaborative efforts of regional resources with local resources and traditional family life and values. The primary goal of the program is for children to be safer in their homes.

Meth and Suicide Prevention Initiative

The Meth and Suicide Prevention Initiative (MSPI) is an program that utilizes the Family Spirit model to provide community support, education and training on the issues related to suicide at villages. This program has 2 –full time staff to coordinate MSPI activities.

SBIRT (Screening Brief Intervention Referral to Treatment)

The SBIRT program is an evidence-based program for planning, organizing, and coordinating alcohol abuse screening activities with all YKHC patient care areas to screen and refer primary care patients to substance abuse treatment, as needed. SBIRT tracks progress SBIRT screenings, referrals made, and referrals completed.

Sobering Center

The Sobering Center is created to divert inebriates from the emergency room and jail to a more adequate and safe place to sober up. It utilizes three full time ER technicians and a manager who is responsible for Sobering Center program design, development, management, quality assurance, and evaluation and reporting

Psychiatric Services

the Yukon-Kuskokwim Health Corporation employs two Psychiatrists on a part time basis. They provide regional psychiatric services by holding specialty clinics in Bethel, as well as, traveling to villages as often as ninety times per year to see clients closer to home. Further, the Yukon-Kuskokwim Health Corporation employs a full time Psychiatric Physician’s Assistant who is located in Bethel to perform psychiatric interventions and medication management for client’s region-wide.

Psychological Services

the Yukon-Kuskokwim Health Corporation employs one Licensed Clinical Psychologist who provides psychometrics and interpretive reports to assist in the provision of comprehensive diagnostics to specific clients in the region.

=== Children’s Specialty Diagnostic Services === provide two mental health professional clinicians who work directly with children to ensure appropriate diagnosis particularly when multiple contributing factors (including abuse issues, FAS/FAE) must be considered.

=== Outpatient Counseling Services === are available in the regional clinic as well to supplement services especially to Bethel residents, when a close relative may be the village counselor, and for the villages who may not yet have a counselor. Services available in the villages include: substance abuse assessments; individual, couple and family counseling; substance abuse outpatient and aftercare counseling; anger management sessions; substance abuse treatment groups and other specialty groups are available. These same services are all available through outpatient counseling services in Bethel. 2. Village Clinicians are Master’s level prepared in Human Services with both experience and specialized training in the fields of Mental Health and Substance Abuse Treatment. They receive culturally appropriate training in supervision of native counselors and work closely with the Village Behavioral Health Aides (BHAs), clients, family and other providers to complete a holistic intake and develop treatment plans for implementation by the village based service providers. This service employs five full time staff members. BHA’s are positioned at many villages to engage community services and make referrals for higher levels of care, as needed.

Residential Services

Crisis Respite Center (CRC)

CRC is a five-bed program that provides emergency support services for individuals experiencing a life crisis so disruptive it cannot be managed in an outpatient setting. CRC provides evaluation, treatment, psychosocial rehabilitation, cultural activities, and referral in a voluntary setting for those individuals needing to be monitored and protected, or those just needing some temporary help. CRC works closely with the Alaska Psychiatric Institute (State Hospital), Emergency Services, Integrated Outpatient clinic and other residential programs. The alternative setting provided by the Crisis Respite Center, while ensuring the client's needs are met closer to their home community, can also ensure the client’s needs are met in a culturally sensitive manner.

Bautisa House

Bautisa House is a home for adults in the region with a severe and persistent mental illness. It is a 10-bed licensed assisted living home. The program serves older and/or more disabled individuals and offers long-term place. Some residents also have a developmental disability. The program teaches them daily living job-seeking and independent living skills as appropriate, and provides access to cultural and community activities.

Malone Home

Malone Home is a 5-bed assisted living home for adults with moderate to severe mental retardation. The team members work closely with DD services to provide appropriate services and habilitation to the residents. The home provides a long term setting for those that live there.

Outpatient Programs

Phillips Ayagnirvik offers two outpatient components. These are both ASAM Level I. Aftercare is offered for those who have completed residential treatment and can access services in Bethel. Outpatient services are provided for those consumers who do not require a residential level of care.

McCann Treatment Center

McCann Treatment Center is a 14-bed facility that houses two programs for male youth ages 10 – 18. The youth from both programs work together with team members to develop their strengths, learn skills to live at home, and reduce symptoms of emotional disturbance and substance abuse. A key component of McCann is a strong culturally relevant subsistence program. Youth are involved in year round subsistence activities, including winter trapping, summer fishing, fall berry picking and spring ice fishing. ==== Residential Psychiatric Treatment Center ==== The McCann Treatment Center (MTC) is a culturally diverse, individualized, strengthens based program that focused on building self-value and meeting the clients’ needs using an eclectic and comprehensive approach to treatment. As a Residential Psychiatric Treatment (RPTC), with a focus on inhalant and substance abuse, treatment includes complete assessments; individual/group/family psychotherapy; psycho-educational groups on multiple topics; recreational and cultural activities; an accredited school with two certified teachers, comprehensive Subsistence Education Program; and life skills training to help each youth gain confidence and self-value. Our state of the art building has a gym for exercise and therapeutic games, a special Men’s House for activities with elders and families, and a steam to relax and process with elders and staff. ==== Behavior Rehabilitation Services (BRS) program==== Eight beds are available for youth placed in the Behavior Rehabilitation Services (BRS) program. The BRS program services clients that are in the custodial care of the Office of Children’s Services (OCS) or the Department of Juvenile Justice (DJJ). Six beds are available for RPTC or Community Referrals. Clients referred to the PRTC beds, as well as the BRS beds, join together in the treatment process and form a therapeutic community. Length of stay is 12-18 months for both the RPTC and Boys Group Home program.

Learning Resource Center

The Learning Resource Centeris a day habilitation program for open to all our residential and community consumers. Psychosocial rehabilitation, skills building, groups, and cultural activities are the focus of the program.

Physical Therapy

The clients seen by Physical Therapy range in age from 2 months to 90+ years of age. A variety of musculoskeletal, neurological and orthopedic cases are seen. Approximately 85 percent of the population seen are outpatients, 98 percent of these patients live on the Yukon-Kuskokwim Delta. Approximately half of the patient population seen in the Physical Therapy Department is from outlying villages. Ten percent of the patients are seen in a sub-regional clinic or off-site facility. Fifteen to twenty-five patients are seen daily, on average. Position titles in the department are Director of Physical Therapy, Senior Physical Therapist, Staff Physical Therapist and Unit Clerk. The Physical Therapy Department operates Monday through Friday, 9:00 am through 5:00 pm, excluding federal holidays.

Village trips are scheduled to the sub-regional clinics in Aniak, Emmonak, Hooper Bay, St. Mary’s and Toksook Bay six times per year. Village clinic trips range from 2 to 4 days each, depending on need.

Respiratory/Cardiopulmonary Services

Services provided by this department include: oxygen therapy, BiPAP/CPAP, ventilator management, intubation assist, intubation per physician request, aerosol therapy with medication, chest physiotherapy, volume expansion therapy, EKGs, cardiac stress tests, peripheral I.V. insertion, pulmonary function testing with DLCO, VO2 uptake, spirometry, methacholine challenges, pulmonary stress testing, ABG procurement analysis, bedside spirometry, cardiac event monitoring, 24 hr. Holter monitoring, sputum induction, airway maintenance, patient education, respiratory breathing techniques , staff education, and are members of the ―Code Blue‖ team. Respiratory therapy services are provided to inpatients and outpatients, OB, ED, Specialty / Peds Clinic, and Outpatient Clinics from infancy to geriatrics. The hours of service for the Department Manager are Monday through Friday, emergency call; the Cardiopulmonary Dept. is staffed with therapists 24 hours a day, 7 days a week.

Anesthesia Services

All levels of anesthesia services are provided for our patient population. The clinical ages range from 2 years old to the elderly. We do not offer elective surgery to patients under two except for short procedures, i.e. myringotomies. We do not offer elective services to patients with American Society of Anesthesiologist (ASA) classification 3 or above except in emergency circumstances. The Department of Anesthesiology provides services for elective procedures Monday - Friday, 8 AM - 4:30 PM. Anesthesia services are available on-call for emergencies and emergent cases 24 hours a day, 7 days a week, including holidays. == Audiology == The primary functions provided are diagnostic, audiology evaluations, specialized auditory pathway evaluations, hearing aid dispensing, and auditory rehabilitation. The audiology program maintains theENT Data Base and schedules ENT Specialty Field Clinics. The site for these services are at YKDRH but patients are also seen in the villages and the audiologist works with hearing impaired students in the schools. Clients range in age from birth to elderly. The hours of operation are Monday - Friday, 8:00 AM - 4:30 PM.

Endoscopy Lab

The Endoscopy Lab provides inpatient and outpatient elective, diagnostic, urgent and emergency care to all ages. Elective procedures will be performed from 7:30 a.m. to 4:00 p.m., Monday – Friday. Emergency or urgent care will be provided as needed 24 hours a day, seven days a week. The unit consists of one procedure rooms for upper or lower GI procedures. Endoscopy procedures may be provided in other units, such as the OR. The same standards of care and practice will apply. Treatment activities may include patient assessment, pre procedural teaching, performance of GI procedures, implementation of pre and post operative physician orders, post procedure assessment and care, verification of discharge instructions, discharge teaching and documentation of care provided. Physicians perform the diagnostic and therapeutic procedures.

Infection Control/Employee Health

Infection control oversees surveillance and monitoring of all the facilities of YKHC in order to identify and recognize hospital acquired infections and other adverse events in the facilities so that prevention and control activities can be identified and implemented to minimize the risk for these events.

Surgical Services/ Operating Room

The Operating Room provides inpatient and outpatient elective, urgent and emergency surgical care to patients of all ages (under age 2 for short procedures) Pre and postoperative areas include Pre-OP room and a Phase 1 and Phase 2 Recovery Room. The surgical suite consists of 1 endoscopy room, 1 operating room with a back up operating room in OB for emergent situations. Elective surgeries are scheduled Monday – Friday from 0800 to 1630 depending on patient needs, physician utilization and staffing levels. Emergency or urgent surgeries are provided 24 hours a day, seven days a week. Prematernal Home

The Prematernal Home provides lodging for pregnant women who have come to Bethel to deliver. The Home offers educational classes, which include breasting feeding, nutrition, diabetes, Healthy Babies and childbirth. Classes offered to clients bring a positive impact to mothers in the Y-K Delta, especially to new mothers who are pregnant for the first time. These courses can reassure women before and after the birth of their child.

Qavartarvik (Hostel)

Qavartarvik (YKHC's hostel) is a part of the Travel Management Center, under Support Services. Qavartarvik is available for use by Natives and Non-Natives who are from the Yukon-Kuskokwim Delta Region villages and have come to Bethel for medical care. Lodging is additionally provided for non-Bethel residents when family members are inpatient at YKDRH and medical escorts who are actively involved in patient care.

Sexual Abuse Response Unit (SARU)

Scope of Service and Staffing
The SARU is a unit staffed and managed by the Emergency Services Department for the purpose of conducting forensic evidence-collection examinations on alleged victims and perpetrators of sexual assault or abuse. The examinations are conducted by specially trained registered nurses – Sexual Assault Nurse Examiners (SANE’S) at the request of either the Bethel Police Department or the Alaska State

Hospital Social Services

Hospital Social Services exists to provide appropriate and timely social services interventions for inpatients and outpatients receiving care at the YKDRH when necessary and/or, requested. The services available include crisis intervention, psychosocial assessments, counseling/therapy, brokerage, advocacy, education, emotional support, complex case management, consultation, problem-solving, investigation and research, specialized discharge planning, and liaison with agencies within the community. Hospital Social Services works closely with a multidisciplinary teamto develop a discharge plan that addresses all needs for post hospitalization care for patients with complex discharge requirements.

Hospital Social Services staff can be called into the hospital during off-hours for deaths and hospital-specific emergent needs. Other issues can wait until the next working day or be addressed by the nursing staff if the concern is minor. The Hospital Social Services department operates 8 hours a day, Monday through Friday, from 8:00 AM to 5:00 PM. The social workers work closely with the nurse care managers in the Outpatient clinics and on inpatient, to provide continuity in patient care.

Troopers depending on whether or not those involved come from Bethel or Delta villages. Counseling and assessment is provided by specially trained clinical social workers. The Sexual Assault Response Unit (SARU) Coordinator is a member of a community-based group who work as a team (Sexual Assault Response Team or SART) to protect and support victims of sexual assault or abuse, prosecute perpetrators, educate the community, and decrease the rate of assaults and abuse within the YK service area.

This issue affects both genders and all ages. An on-call system is maintained to assure availability of a competent examiner with coordination of services supervised by the Emergency Department Nurse Manager. The on-call system eliminates positive variance and the emergency room staff handles negative variance.

Long Term Care Facility

A Long Term Care Facility is currently under construction, with a ground breaking ceremony that occurred in August 2011. This long awaited building will be an eighteen (18) bed facility that will serve the elder population in our region that meet the needed criteria. The anticipated opening of this facility will be October 1, 2013.

Morgue

There are no funeral homes in Bethel. For those patients that die at YKDRH, their bodies are held in the hospital morgue until transported to the village or buried in Bethel. Law Enforcement (Bethel Police Department and State Troopers) also bring bodies to the hospital morgue, awaiting transport to the State Medical Examiner’s Office in Anchorage, for an autopsy. Priority use of the morgue belongs to the hospital. Law Enforcement is responsible for finding their own alternate storage space when the morgue is full. Families of the deceased visit the morgue for viewing and preparing the body for burial. The morgue can accommodate two bodies and is available 24 hours a day, 365 days a year. No autopsies are performed in Bethel. . The hospital Social Services is available to assist as needed 24 hours a day, 365 days a week.

Medical Nutrition Therapy Department

Medical Nutrition Therapy provides nutrition assessment and education for both inpatients and outpatients. Through consults/referrals from health care providers, a Registered Dietitian provides dietary consultation. Patients range from infants to elderly. The Registered Dietitian also oversees the development and implantation of the hospital’s inpatient menu. The hours of operation are from 8:00 am to 5:00 pm, Monday thru Friday. On weekends and holidays the Registered Dietitian carries a pager, and is available for nutrition consults as identified by the physician.
