Medical Nutrition Therapy
Medical Nutrition Therapy provides nutrition assessment and education for both inpatients and outpatients. Through consults/referrals from health care providers, a Registered Dietitian provides dietary consultation. Patients range from infants to elderly. The Registered Dietitian also oversees the development and implantation of the hospital’s inpatient menu. The hours of operation are from 8 a.m. to 5 p.m., Monday through Friday. On weekends and holidays the Registered Dietitian carries a pager, and is available for nutrition consults as identified by the physician.
Prescription Produce Programs (PPP)
Facilitated by Bethel Community Services Foundation.
PPP 1.0 for patients with pre-diabetes, diabetes mellitus, or gestational diabetes.
PPP 2.0 for adult Medicaid enrollees with "Food Insecurity" (Z59.41) on problem list and NO diabetes diagnosis.
- Order "Refer to Food Insecurity Produce Program."
- Patient must sign correct consent form and staff must email to prescription_produce@ykhc.org.
