At time of referral, the patient and referring clinician will complete the HHS Sterilization form accessible here.
The referring clinician orders “Refer to Surgery-Vasectomy.”
Patient will receive phone call from the surgeon to discuss procedural consent, reiterate details of procedure, and affirm patient’s desire for permanent sterilization.
The patient will later receive phone call from case manager to schedule procedure.
On day of procedure, the patient presents to Surgery Department (across from cafeteria).
