Epinephrine for cardiac arrest without an IV

Literature review (May 2025): In several studies with small sample sizes, there is evidence for non-inferiority of IM epinephrine (compared to IV) in cardiac arrest in rat and piglet models. There have been several feasibility studies that use IM epinephrine in human out-of-hospital cardiac arrests. These study protocols use doses of 5 mg IM and generally follow the IM doses with IV/IO doses. These studies are performed in resource-rich locations in which full ACLS is feasible soon after IM administration.”
